The Community Service Log is available on the Court's website or by calling the Court during normal business hours at 720-977-5400.  The community service log must be filled out completely and be legible.  All hours will be verified upon filing with the court, so all contact information on who you worked for is essential. You must ensure dates, hours, and work done are indicated, total hours are tallied, and the form is signed before submitting the proof to the court.
