How do I request my patient care report?

Patient Care Reports may be requested at the Thornton Fire Department Administrative Offices at 2300 Thornton Parkway, Thornton, CO 80229, anytime during normal office hours (Monday through Friday, 8 a.m. to 4 p.m.). A standard HIPAA release form will need to be completed in order to receive any medical records. Please call the Administrative Offices at 303-538-7602 for more information.
